The Case Management Program is available to students who have been referred by SFUSD Staff, family,& community partners . Students enrolled in case management are able to utilize weekly check ins for additional academic and social-emotional support. This includes academic tutoring, help with self-advocacy, referrals, and connection to community resources. Youth in this program will also have access to referrals to Intensive Case Management services by qualified staff which includes counseling and restorative interventions for youth and families. Meetings occur weekly or every other week and are scheduled in advance by the Case Manager and student. During these meetings, Case Managers help students identify personal and academic goals, develop a plan to achieve these goals and connect students with school and community resources.
